In 2022, West Virginia had the highest rate of age-adjusted fentanyl overdoses of any state, at 67 deaths per 100,000 people, 42.3% higher than Delaware, with the next highest rate of 47.1. West Virginia was home to seven of the 10 counties with the highest fentanyl overdose rates, based on 2022 county-level CDC data.
South Dakota had the lowest rate of fentanyl overdoses, with 4.6 deaths per 100,000 residents.
